Your agreement with Queensland Health regarding the use of the site will always include, at a minimum, the terms and conditions set out in this document.
In order to use the site, you must first agree to the Terms of Service. You may not use the site if you do not accept the Terms of Service and you will need to contact Payroll Services on 1800 239 074 to discuss alternatives.
You can accept the Terms of Service by selecting ‘Accept the Terms of Service’ when completing your registration for Streamline, or clicking continue when prompted from within the site.
Before you continue, you should print or save a local copy of the Terms of Service for your records.
Queensland Health will only provide site access to current Queensland Health employees.
You acknowledge and agree that if Queensland Health disables access to your account, you may be prevented from accessing the site, your account details or any files or other content which is contained in your account.
In order to access the site, you are required to provide information about yourself (such as identification and contact details) as part of the registration process for the site. You agree that any registration information you give to Queensland Health will always be accurate, correct and up to date.
You agree to use the site only for purposes that are permitted by (a) the Terms of Service and (b) any applicable law, regulation or generally accepted practices or guidelines.
You agree that you will not engage in any activity that interferes with or disrupts the site (or the servers and networks which are connected to the site).
You agree that you are solely responsible for (and that Queensland Health has no responsibility to you or to any third party) any breach of your obligations under the Terms of Service and for the consequences (including any loss or damage which Queensland Health may suffer) of any such breach.
You agree to use the site in accordance with the lawful and ethical standards of conduct and appropriate workplace behaviour while applying the principles of integrity and impartiality, promoting the public good, and committing to the system of government, accountability and transparency under the Code of Conduct for the Queensland Public Service which applies to all Queensland Health employees.
You agree and understand that you are responsible for maintaining the confidentiality of your password associated with your site account.
Accordingly, you agree that you will be solely responsible to Queensland Health for all activities that occur under your account.
If you become aware of any unauthorised use of your password or of your account, you agree to notify Queensland Health immediately at firstname.lastname@example.org.
Queensland Health recognises the importance of managing personal information to respect the privacy of staff.
Queensland Health is required to comply with the privacy principles contained within the Information Privacy Act 2009 (IP Act) which includes the nine National Privacy Principles (NPPs) and provisions regarding contracted service providers and the transfer of personal information out of Australia. These are essentially rules about how Queensland Health must handle all personal information including collection, storage and maintenance, access and amendment, openness and use and disclosure of personal information. The principles are supported by a range of Queensland Government and Queensland Health policies.
Queensland Health is collecting your personal information to facilitate site registration. Once registration has occurred, you will be able to access certain information when you log in to Streamline, including your pay advices, PAYG payment summaries, overpayment details and equality of employment opportunity (EEO) information.
The personal information stored in Streamline may be used or disclosed by Queensland Health in accordance with the NPPs, which includes but is not limited to use or disclosure in the following scenarios:

Queensland Health provides you with your pay advices and payment summaries electronically via this site. You agree to receive your pay advice each fortnight, and your payment summary each year, electronically through this site.
The information provided on the site is for your information purposes only. Queensland Health does not warrant nor represent that information contained on the site is complete, current, reliable and/or free from error. The site is not intended to be, and should not be relied upon as, the ultimate and complete source of payroll information.
To the maximum extent permitted by law, Queensland Health accepts no liability in relation to or arising out of your use of the site.
If Queensland Health’s liability in relation to a breach of these Terms of Service, statutory cause of action, common law or tort (including negligence) in any way cannot be excluded, then such liability is limited to $1000 and Queensland Health will not be liable for any indirect or consequential loss or damage, notwithstanding anything else in these Terms of Service.
Queensland Health may make changes to the Terms of Service from time to time. When these changes are made, Queensland Health will make a new copy of the Terms of Service available to you on the site.
You understand and agree that if you click continue after the date on which the Terms of Service have changed; Queensland Health will treat this as acceptance of the updated Terms of Service.
